FACILITATE PATIENT'S COMMUNITY NEEDS
Facilitates the patient's daily living by helping them gain access to any problems they may be having that don’t allow them to fully live their lives. For example; getting food stamps to buy groceries if they need it, enrolling in night classes if they wish to learn another language, assisting in automating bill payments, solving any issues with their citizenship/residency paperwork.
